Time to Renew Health Paperwork for 2024 - 2025

The first day of school is coming up and it's time to renew health paperwork!

If your child has a known health conditions that requires medication at school you will need to complete and submit paperwork prior to the start of school. The paperwork does require a doctor's note.

This includes:
Consent to Administer Medication (if the condition needs medication administration such as a prescribed Epi-Pen or emergency seizure medication for a prolonged seizure)
Seizure Action Plan
Allergy and Anaphylaxis Emergency Plan

A new Authorization is required at the beginning of each school year and any time there is a change in medication directives (such as change in medication, dosage, timing, or frequency).
